• Independent Volunteer Activity Form (use this if you have completed an independent volunteer project to earn credit for your community service hours)
  

• Service Project Evaluation Form (please use this to provide your feedback; when complete, please give to the Project Leader after event or to Chapter Leader at monthly chapter meeting)

• Project Sign-In/Out Form (Project Leaders will bring this to each service event for you to sign; if parent/guardian will not be attending with teen, please fill out prior to event with parent/guardian signature.  Teen must give signed form to Project Leader prior to event)
  

Below are additional waiver forms required by various agencies (please print and have a parent/guardian sign prior to event if a teen will attend a project for one of the following agencies without parent/guardian present) –

Pt Loma Native Garden Form #1 – please sign for children under age 18 attending without parent/guardian

Pt Loma Native Garden Form #2 for adults attending (in case adult would like to review and sign prior to attending)
